I confirm and understand this property is a non-smoking property and that failure to comply with this non-smoking policy will result in a $500 fine which will be applied to the credit card used for my pre-authorization.

By signing below, I understand and agree to the following policies and regulations both listed below and stated at the signing reception office in their entirety and agree I take full responsibility and waive City View Inn and all connected parties from all liabilities for myself and all persons included within my party including all persons visiting myself or my party. This property is privately owned and managed. The owners, management and staff are not liable for all damages or theft of private property, accident, injury, including slip and fall or theft (of any sort) pertaining to myself or my party. I agree I will not use this property in its entirety as a permanent primary residence and understand all privately addressed mail will be returned to sender. I understand this property is a smoke free property. The designated smoking area has been identified to myself and I take responsibility to inform my party. The use of marijuana is strictly prohibited without a province of Ontario documented license. The use of medical marijuana must be identified at the time of check-in. City View Inn is a pet free property (Including service animals) as we guarantee an allergen free environment for all guests and staff I take responsibility to educate my party and all visitors regarding all policies on this property. A zero-tolerance policy is in effect always. Management and staff have the right to remove all guests and visitors from this property at any point in time without refund. Examples are as follows: • Disturbance to owners, management staff and neighboring properties • Exceeding total registered party stated at check-in • Damage or theft on private property or neighboring properties • Aggressive, harassing or violent actions both verbal and physical • Having pets or smoking on property except where designated • Unsuitable room care conditions which are at managements discretion I pre-authorize $200.00 cdn and understand these funds may be kept without notification if policies are violated. For Damages in excess of $500.00 cdn – I authorize City View Inn to process the registered credit card below without notification or dispute.
